According to his post on X.com, Johnson’s stack is as follows: 

  • Acarbose: 200 mg/day 
  • Tadalafil: 2.5 mg/day 
  • Candesartan: 8 mg/day
  • Jardiance: 10 mg/day
  • Repatha: 140 mg/biweek 
  • Thyroid Extract: 60 mg/day 
  • Levothyroxine: 100 µg/day
  • Minoxidil: 3.75 mg/day  

Capitalist and longevity poster-child Bryan Johnson is known for spending millions of dollars on improving his health and longevity. His slogan is “Don’t Die,” which is the goal of his extensive anti-aging routine. He recently took to X.com, showing his latest lab results, saying, 

“I may have the best comprehensive biomarkers ever measured. Here are the receipts. There’s a title for the fastest person in the world. And the richest. But up until now, no one has become the healthiest. I share my protocol so you can beat me.”

He highlights several results, including his testosterone levels, which he says are excellent despite his largely vegan diet. He attributes his high vitamin D and B levels to his hyperbaric oxygen chamber sessions. At the end of the thread, Johnson reveals his “Longevity Therapeutics,” the list of pharmaceuticals he apparently associates with his healthy biomarker readings. 

Johnson’s Longevity Therapeutics 

Acarbose (200 mg Daily) 

Acarbose is a prescription anti-diabetes drug that minimizes blood glucose levels by inhibiting an intestinal enzyme that would otherwise speed up the digestion of carbohydrates. Johnson says, acarbose “helps blunt post-meal blood sugar spikes, improve insulin sensitivity, and has been shown in long-term studies to reduce cardiovascular events and extend lifespan in animal models, making it a valuable tool for metabolic health and longevity.” 

Tadalafil (2.5 mg Daily)

Tadalafil is a medication prescribed for the treatment of erectile dysfunction that relaxes blood vessels to increase blood flow. Johnson says, tadalafil “enhances endothelial function, reduces inflammation, and potentially protects against age-related cardiovascular and metabolic decline.” One of the studies Johnson may be referring to is one showing that tadalafil reduces the risk of all-cause mortality, heart attacks, strokes, and dementia. 

Candesartan (8 mg Daily) 

Candesartan is prescribed for the treatment of high blood pressure and heart failure. Like tadalafil, it relaxes blood vessels. Johnson says there is “evidence that [candesartan] reduces blood pressure, protects against heart failure progression, preserves kidney function, and possibly reduces brain inflammation.” A study showed that intranasal administration of candesartan reduces brain inflammation in a mouse model for Alzheimer’s disease, which may be one of the studies Johnson is referring to. 

Jardiance (10 mg Daily)

Jardiance is the brand name for the anti-diabetic medication empagliflozin, which blocks the kidneys’ glucose retention by inhibiting sodium-glucose cotransporter 2 (SGLT2). Johnson says there is “good evidence of SGLT-2 inhibitors as a longevity intervention and calorie restriction mimetic from animal experimentation.” The inhibition of SGLT is associated with decreased risk of heart disease, kidney disease, liver disease, cancer, and neurodegenerative disease.  

Repatha (140mg/mL Every Two Weeks), Thyroid Extract (60 mg Daily), Levothyroxine (100 µg Daily), and Minoxidil (3.75 mg Daily)

Acarbose, tadalafil, candesartan, and Jardiance are under investigation as potential longevity therapeutics for healthy individuals. These drugs can potentially mitigate chronic disease and delay death, which is what Johnson is aiming for. However, he uses Repatha, thyroid extract, levothyroxine, and minoxidil for their primary indication. 

As such, for cholesterol optimization, Johnson uses Repatha — the brand name for evolocumab, a drug prescribed for lowering cholesterol. Additionally, he uses yet another blood vessel relaxer (vasodilator), minoxidil, to counteract hair loss. Lastly, being diagnosed with hypothyroidism at the age of 21, Johnson also takes Armour Thyroid thyroid extract and levothyroxine. 

Johnson Can’t Actually Pinpoint What’s Improving His Health 

At least at one time, Johnson was taking over 50 supplements. He also follows a strict diet, exercises routinely, and obsessively maintains his sleep schedule — all known to contribute to better health and longevity. With so many variables, it is difficult to discern what exactly is improving Johnson’s biomarkers. Still, it can be argued that his cholesterol and glucose levels are ideal because he is taking cholesterol-lowering and glucose-lowering drugs. With that being said, at this time, it is unclear which of the drugs listed above are necessary for healthy individuals who diet, exercise, and sleep well.
